In order to add a script into your ISN you will need to create "Label" control in your order form. The image below shows how the label will appear in your order form.

First you will need to navigate to the "Office Setting" area in your ISN by going to "Settings" and clicking on "Office Settings".

Once in "Office Settings" navigate to "Customize Order Form" on the right hand side under where it says "Inspection and Fee Settings".

You will then need to find the section where you want to add your script to and click on the pencil on the right hand side to edit the "Controls" for that section.

Next click on the green "Add A Control" button.

In the control type drop down choose the type called "Label" towards the bottom of the list.

Then add the question from the script that you want to ask into the Name, Description and Text Value boxes and click on the blue "Add Control" button. The "Text Value" box will allow you to format how the text appears on the order form.

After creating the control find it in the control list of that section and move the label to the appropriate location by clicking and holding the 3 lines on the left hand side and dragging.

You have now added a piece of your script to the order form and can repeat it as needed.
